In total, there are 37 yes/no items, 5 open-ended items, and 28 reinforcers to choose from. Best of all, this bundle includes the interview in two different formats - Google Docs and Google Forms!

The FBA PARENT INTERVIEW includes 21 questions that assess a parent’s perspective regarding his/her child’s relationship with others, emotional well-being, general behavior, and factors that promote positive behavior in his/her child. In addition, a parent can choose between 26 descriptors to identify the adjectives/behaviors that describe how his/her child responds most of the time. Parents don't have an email address or access to a computer? No problem! Instructions are included explaining how to text the form to a parent to be completed on a cellular device. 

The FBA TEACHER INTERVIEW consists of 18 multiple choice/checklist items and 14 open ended questions that guide teachers in: creating an operational definition of the primary behavior concern (including specifics about the frequency, intensity, and duration); identifying the antecedents and consequences of the behavior; determining the probable function of the problem behavior; and sharing their thoughts regarding effective/ineffective strategies, the root causes of the student’s problem behavior, and the student’s overall strengths/weaknesses.
